Step 1: Complete the Intake Form for Your Location

Begin by filling out our secure intake form. This helps us understand your needs, goals, and how we can best support you.

Step 2: Schedule an Intake Call

Once your intake form is submitted, return to this page to schedule a 15-minute intake call with a member of our staff. You will be able to choose a time that works best for you from our available openings.

Step 3: Attend your Intake Call

Please note, you will be expected to call the office at 203-283-7027 at your scheduled time. During your call, a staff member will review your intake form, answer initial questions, and discuss possible next steps.

Step 4 (Optional): Schedule a Consultation with Our Clinical Director or our Occupational Therapist

If you would like more in-depth guidance, you may choose to schedule a 60-minute consultation with our clinical director or our occupational therapist for an additional fee. This consultation may be helpful if you want a more comprehensive clinical perspective, need help determining the best treatment approach, or have specific concerns you would like addressed.
